All student over-the-counter and prescription medications must be accompanied by the appropriate medication forms. No medications will be accepted at your child's school without the proper documentation. New forms must be submitted each school year. All medications must be picked up by an authorized adult at the end of each school year. Medications not picked up will be disposed of. For student health forms click this link.

Interpretation Services Request

Martinez Unified School District celebrates the home languages of each of our students. We recognize that families may need interpretation services in order to access services and communicate with school district personnel. We can provide interpretation services when needed. Pick-Up/Drop-Off Change

I wanted to make you aware of a change in the traffic flow in the District Office Parking lot located at 921 Susana Street. Currently, some of our parents use this lot to drop their children off so they can walk up the hill to our campus from Susana Street. However, parents access the lot from both the Susana Street and the Henrietta Street entrances and pass through the lot as they drop off, and/or pick up, student(s). This traffic pattern has become a concern for our neighbors on Henrietta and in some instances a concern for employees working at the district office.  


In an attempt to resolve the traffic issues on Henrietta and the safety issues for our employees here at the district office, on January 8th, 2018, the Henrietta access gate will be closed.  The parking lot can still be accessed from Susana Street, and you will still be able to pull into the lot, and make a full circle with the traffic flow until you exit again on Susana Street. The only chance is that you will no longer be able to access the lot or exit the lot from Henrietta Street. We ask that when dropping your student(s) in the parking lot that you drive slowly and be mindful of employees needing to park and cross the lot to access the the employee entrance.

Incoming 7th graders need Tdap before they can begin 2018/2019 School Year

Sixth graders need a Tdap shot to start 7th grade. The booster shot will help to protect your child against pertussis, commonly known as whooping cough. If your child has had the shot, please submit proof to the MJHS Student Services Office as soon as possible.  A Tdap shot given at 7 years or older satisfies the state requirement.


For your convenience, you may fax your child's shot record to the MJHS Main Office at 335-5829 or send it by email to shotrecords@martinez.k12.ca.us

Student Medications

All over-the-counter and prescription medications kept at school must be picked up by an authorized adult by the last day of school. Unclaimed student medications will be discarded. Parents must bring in new medication authorization forms each school year with their student’s medications. No medications will be accepted at your child’s school without the proper documentation. Student medication forms are available in the school office and on the school and district websites.
